Request to Register to Vote outside Québec

1. One document must mention your name and your date of birth:
   • canadian passport;
   • health insurance card;
   • citizenship certificate;
   • birth certificate.
2. The other document must indicate your name and the address of your domicile in Québec:
   • driver's license (front and back);
   • electricity or phone bill;
   • lease;
   • home or auto insurance statement.

One of the supporting documents must include your signature.

The information you provide on this form is required to register to vote outside Québec. All information collected is mandatory, unless otherwise indicated.

This information is confidential under the Act respecting access to documents held by public bodies and the protection of personal information. It will only be used for the purposes set out in the Election Act. Élections Québec may retain this information for up to one year after the date you provided for your return to Québec, subject to the rules set out in its retention schedule. The institution takes the necessary security measures to ensure the protection of personal information that it collects, uses, communicates, retains or destroys.
